Follow the Medicare appeal levels
Redetermination: Submit level-1 request exactly as directed in your Medicare notice.
- Level 1: Ask for redetermination using the instructions in your Medicare notice.
- Level 2: If denied again, ask for reconsideration by a Qualified Independent Contractor.
- Level 3: If needed, ask for a hearing with the Office of Medicare Hearings and Appeals.
The rule to remember
"Same file. New deadline every level."
Medicare Advantage and Part D follow the same structure but may use different level-1 naming and deadlines.
